Service Record Review

Request review of an approved service record.

FWF can confirm facts supported by reviewed records for an approved activity. An institution, employer or other authority decides whether the service meets its requirements.

Identify the activity, not the people it supported

Use the current contact form to provide the activity or record reference, if known, and explain whether you need a review, correction or authorized confirmation to another organization. Choose Volunteering & Students as the topic. Please do not submit private information about people who received support.

For a correction, describe the entry or fact you believe needs attention and the change requested. A brief initial question is enough; do not upload identity documents or someone else’s private records.

Permission and review come before disclosure

If you are requesting information about someone else’s participation, explain your role and purpose. A request or reference number alone does not authorize disclosure. We may need the participant’s permission and appropriate identity and authority checks before sharing information with another party.

Service records describe substantiated participation. They do not automatically establish academic credit, professional accreditation, employment or acceptance by another institution.

Agree requirements before work begins

School, court, employer or other service requirements should be reviewed before an activity begins. Only approved activity and records that can be substantiated can support a confirmation; an inquiry does not create verified hours or a completion record.
